The 18-year-old, who has not been named, had been surfing off Freshwater West Beach, near Pembroke, in West Wales yesterday.
St Govan's Coast Rescue Team was sent to help get the teenager out of the water after he dislocated his shoulder.
The team was joined by paramedics from the Welsh Ambulance Service.
Paramedics began treating him and took him to Withybush Hospital by land ambulance.
The wind swept and exposed beach is considered one of the most consistent surf spot in Wales with waves often reaching six to eight feet in height.
Elsewhere, the coastguard went to the aid of another Shropshire man who got into difficulties in an incident off the Welsh coast.
The 42-year-old man had to be helped ashore after becoming separated from his companion while canoeing in "adverse conditions" in the sea near Fishguard, Dyfed.
The man, from Whitchurch who has not been named, was helped ashore on last Monday off the coast at Cwm yr Eglwys.
Jane Griffiths, spokeswoman for Milford Haven Coastguard, said last week the number of incidents dealt with was normal for the time of year apart from yesterday when emergency services had 10 incidents in one hour.
