Myddle CE Primary School, near Wem, has been handed a notice to improve.
The school, which has 64 pupils aged four to 11, has been told to improve teaching standards, pupil progress and its monitoring and evaluation procedures.
The warning comes after an Ofsted inspection in February.
In her report, lead inspector Sarah Bentley, said: "The school's effectiveness is not satisfactory because its leaders and managers have not made sufficient improvements to the school's performance since the last inspection. Although achievement overall is satisfactory, progress is variable throughout the school and not all pupils make consistently good progress.
The report acknowledges that some improvement has been made in recent terms.
Wendy Hallam, chairman of governors, said: "The governors are aware of the development needs raised.
"They will continue to address these through the school development plan, which the report recognises is already having a positive impact."
