The 21-year-old, from Donnington, has also been charged with failing to provide samples for drink or drug driving tests.
He is also accused of failing to stop for police and driving document offences.
The alleged offences are said to have taken place on Wednesday.
The man, who has not been named, will appear at Telford Magistrates Court on June 22.
