The care home on Keepers Crescent in St Georges provided assisted living for adults with learning disabilities, but it closed in 2019.
In May last year, charity Bournville Village Trust revealed it was looking at transforming the building into six individual rooms providing short-term accommodation under a property guardianship model.
Now, the property has been listed for sale by Halls Commercial for offers in the region of £550,000.
A spokesperson for Halls said: "The property comprises of a substantial detached six-bedroomed former care home that would lend itself to this continued use or as a residential development opportunity, subject to any statutory consents and has significant potential.
"The property on the ground floor is arranged to provide an entrance porch, inner hallway, shower room, utility room, commercial kitchen, living room, two inner lobby areas and a bedroom for the manager of the care home.
"The first floor provides a landing area, inner hallway, toilet, bathroom and six bedrooms. The internal arrangement of the property means that there are two front doors to the property to the front elevation as it fronts onto Keepers Crescent therefore allowing potential separation."
The former care home also has off-road parking and a garden to the rear.
