Shropshire Council nursing home beds contract is on offer
Shropshire Council is looking to obtain up to 10 nursing home pathway beds to help place less people in care and hospital.
The Discharge to Assess beds will be based in care homes and will provide support for older people who have been discharged from hospital, through confidence building, recuperation and regaining independent living skills.
In addition to the beds, other initiatives the council has launched include bespoke night time support, carer support post hospital discharge, and four new ‘independence’ units located within housing developments.
Tanya Miles, Shropshire Council’s head of adult social care, said that the projects were aimed at preventing bed-based care in hospitals or nursing homes.
She said: “Shropshire Council has launched a series of initiatives to ensure those in need of support remain at home rather than in a nursing or care home.
“The programmes form part of the sector’s continuous commitment to place less people in care and hospital settings and more in the community.
“The projects are also designed to prevent the need for bed-based care either in hospital, in a community hospital or nursing home setting – whether that’s helping to avoid people entering hospital or assisting them to return home as quickly as possible.
“As part of the additional money we are procuring some additional nursing beds to facilitate discharge from hospital.”
“Discharge to Assess (D2A) beds form part of a wider programme of initiatives that have been developed by Shropshire Council and Shropshire Clinical Commissioning Group to support older people who have been discharged from hospital. The ‘home first’ philosophy is key to D2A beds.
“D2A beds will provide a safe and caring environment for people to recover further from hospital treatment. The focus in D2A beds is on short stays, where recuperation, regaining confidence, health, and independent living skills are key. Through this expression of interest the council is looking to secure D2A beds in local care homes that will work in partnership with social workers, GPs and therapists, to make sure discharges from hospital are timely and effective and achieve positive outcomes for older people.”
The pathway bed contracts will be for an initial term of nine months from when the first bed becomes occupied.
The deadline for returning the expression of interest is 12noon on September 8.
